I have never known a manufacturing business where the essential production information was completely up-to-date.

One reason is that the workforce discover how things can be done but fail to pass that information on to those who have specified how it is to be done. As long as the output is satisfactory, nobody worries very much about keeping the records straight. Wrong, I know, but it does happen, even in organisations that have good quality-assurance programmes.
